Skylos Sports Medicine offers primary care veterinarians no-charge case consultations, whether or not you're planning a referral to one of our hospitals. Requests should include a brief history and digital radiographs, which you can submit via the online form below. Upon receipt, a Skylos veterinarian will respond within 24-28 hours.

While radiographs may be reviewed by Skylos as part of a case consultation, please keep in mind that our surgeons are not radiologists and cannot provide a formal radiographic interpretation for inclusion in a patient file.

    Records and Communication Policy

    BluePearl Pet Hospital is part of your pet’s continuum of care, and we may provide your family veterinarian, any specialists, and/or a reviewing body information regarding your pet’s treatment and/or condition. By signing below, you are authorizing BluePearl Specialty and Emergency Pet Hospital to share your pet’s medical record with a third party (such as your family veterinarian) or its agent necessary for us to provide continuous veterinary care to your pet. You agree that we (or our agents) may send you communications to your contact information provided above regarding your pet.
